クリーニングにも色々あって、デザインナイフで砂つぶひとつひとつを弾き飛ばすようなクリーニングもあるし、中身を穀物粉に変えたサンドブラスターでゔぁーっとやっちゃうクリーニングもある。もちろんエアスクライバーと双眼顕微鏡のコンボも。歯ブラシがしがしクリーニングもあるし、酸処理もある。

今年度から、放課後等デイサービスのシステムが変わり、専門職を沢山配置するためには、1型と言われる事業所にならないといけません。強度行動障害という、かなり問題ある表現をされる、でも、頑張ってる子どもたちが50パーセント以上いないといけないと、今年2月くらいに急にフワッと言われました。

正直、強度行動障害は、1980年台後半にキリン財団の研究から言われ始めたローカルな言葉です。重度の知的障害がある入所の方々を対象にした調査研究でした。ウチの事業所の療育のチーフも大学院で強度行動障害の研究をしていたので間違いなく。
厚労省がそれを安易に児童に適用してきました。

強度行動障害の指標は、あまりに知的障害のない発達障害児の臨床像と離れ、解釈すれば当てはまりはしますが、保護者にしてみれば、受け入れ難いような項目でした。でも、国の決まりですから、仕方ありません。指標通りに児童が区分分けされることになりました。3月末くらいの段階です。

福岡県は政令市を除き、県が事業所を指定し、市町村が受給者証を発行するシステムになります。受給者証とは、児童が放課後等デイサービスを利用する証明書で、健康保険証みたいなものです。そこに、強度行動障害児童（指標該当児童）と記入されることになります。

4月の放課後等デイサービスの新システムに対応するためには、それぞれの児童が該当児童（強度行動障害児童）か、非該当かを区別する必要がありました。しかし、3月末に市町村に降りてきた話で、市町村は混乱しました。我々…一番には児童が巻き込まれました。

宗像市市職員について、上司に苦情を言ったところ、驚くべきことに、その人は社会福祉士ということがわかりました。
障害担当課は、「専門家」ということで、彼を上げ奉ってたようですが、あまりの専門性の低さに呆れ、専門家の前に、公務員としての常識を身に付けるよう、上司に頼みました。

我が事業所の管理者は新システムをしっかり精査し、ちゃんと国に報告しましました。

つまり、請求の〆切前から、自分の失敗が分かっていながら、加算を付けると大変なことが起きると分かっていながら、事業所に告知していなかったのです。
国保連の担当者は、とにかく明日までに強度行動障害の加算を外して請求すれば、基本的な報酬は入ると提案してくれました。
