本来なら想定外・規格外の対象物を除去するには、その大きさや重量を調査し、（本来の引き抜き工法ではなく）掘り起こし工法等に切り替えて適切に処理する必要があったが、当初の段階から作業者は「かぶが抜けなかった理由」の一つが「自分がその上に乗っているから」という事にも気付いていなかった。

報告者である農夫から詳細な聞き取り調査を行ったところ、作業内容についても幾つかの重大な問題点がみられた。
まず「かぶの定義」つまり寸法や質量的な基準が明文化されていないため 作業者は普段の手順通りにこれを引き抜こうとしたが、自分の片足が「かぶ」を踏んでいる事にも気付いていなかった。

この「大きなかぶ」の件は 幸い負傷者が無かった事もあり、当初はヒヤリハット報告に埋もれていたのですが、しかし報告内容とハザードの大きさを鑑み、改めて重大インシデントに位置付けて再調査する事となりました。

人力による「引き抜き工法」で対応可能なかぶのサイズにも上限がある事は想像に難くありませんが、この事例では作業手順として想定していない「規格外の大きさのかぶ」が存在した事も一因ですから、もしかぶのサイズについて作業手順書に明文化されてないなければ、この機会に改めた方が良いでしょう。

大きなかぶを引き抜く工程においては（結果はさておき）、作業手順が適切とはいえませんでした。
引き抜くかぶのサイズが、普段の作業手順で想定しているよりも大きく上回っている事が確認された時点で、より適切な作業計画に見直す必要があった筈です。

石に突き刺さった聖剣を引き抜く場合であれ、大きなかぶを人力（+犬猫鼠）で無理やり引き抜く場合であれ、まず初めに注意すべきことは「抜けるか抜けないか」だけでありません。
力任せに引っ張って抜けた場合あるいは一部が破断した場合に、その反動で後ろに勢いよく転倒する危険性を想定すべきです。
