POCUSっていうのはポイントオブケアの超音波で、蘇生中に有効そうな心電図が見えても拍出がない場合もあるので有効な拍出のあるものかどうかを確認するのにマニュアルでの脈確認がよいか超音波での確認がよいか、とにかく速いのははどちらかっていう議論が救急ではあって。

理由は、脈拍確認して迷うと、心マが遅れて不可逆的損傷を与えかねないからです。
2000年のガイドラインには、脈拍触知は入ってましたが、2005年の改訂で脈拍触知は外されました。

病院なら脈とか、医療従事者なら脈とかもか書かれてるけど、それもどうかと。モニター装置状態ではない人ならCPRを始めて最初のショックを判断する時に波形確認で胸骨圧迫を止める。その時に脈を期待できる波形ならなら有効な拍出があるかどうかを確認する為に初めて脈確認が出てくるくらいよ。

去年歩道に倒れていたおじいちゃんをひとりで救助した体験からずっとやらなきゃって思ってた（意識はあって周りに誰もいなかった）から長かったけどしっかり勉強してきた。
